Here we go...all the answers have been collected, checked, cross referenced, double checked and cleared through the Ministry of Magic and the Hogwarts School Governors and I can now announce the results...
Firstly, the questions again, but this time with the answers and page locations. I've checked all the pages with the UK versions of the books, so I'll state the UK page numbers here.

In Harry's 4th year, what was his course schedule from Monday-Friday except Wednesday, before the break and lunch?
Monday:Herbology, (page 171 UK GoF chapter 13)
Tuesday:History of Magic (page 304 UK GoF chapter 20)
Thrusday:Transfiguration (page 336 UK GoF chapter 24)

What happened on the 12th of sep 1991 ?
Harry becomes a Seeker (page 112, PS (UK) chapter 9)
The year is confirmed by The Deathday Party in Harry Potter and the Chamber of Secrets, where it is confirmed that Nnearly Headless Nick died in 1492 (1992 therefore being the 500th Anniversary). The event of harry becoming Seeker occurs in the second week in September in the year prior to Nearly Headless Nick's 500th Deathday, and is on a thursday, whch makes it the 12th September 1991.

An International Warlock Convention (Chamber of Secrets, UK version, pg 113, chapter 9)
4) [i]what medical ailment have ron, Harry and Cedric all had in common?
They have all been hurt by a dragon
Ron by Norbert the Norwegian Ridgeback in Hagrid's hut (page 173 Philosophers Stone (UK) , Chapter 14).
Harry was cut by spikes from the Hungarian Horntail's tail during the first task in the Triwizard Tournament (page 310 Goblet of Fire (UK) chapter 20).
Cedric was burnt by a Swedish Short Snout during the first task by the Swedish Short Snout ( page 314 GoF (UK), chapter 20).
Sooooo...who scored how many points? There were 2 points available per question, but 4 points for the hidden bonus question.
On Question 1,
Ravenclaw had all four lessons correct, with page numbers for each lesson, so score the full
2 points.
Gryffindor had all four lessons (and also gave details of lessons after the morning break) but only mentioned a page number for the first question. Taking into consideration the detail provided, I've awarded
Gryffindor one point, but also awarded
Hufflepuff 2 points because of Gryffindor not giving all page numbers, so technicallyunder the new rules Gryffindor only partly answered this one.
Slytherin provided the correct answer and page number for Monday (including the fact that
Care of Magical Creatures was after the morning break), but did not provide details for Tuesday, Thursday and Friday. Therefore, I've awarded
Slytherin half a point and
Hufflepuff 2 points again, because of setting a question which Slytherin didn't answer absolutely 100% correct.
Question 2 was the
hidden bonus question! worth 4 points. Unfortunately for Hufflepuff, all 3 other houses correctly stated that Harry was made Seeker, and provided page numbers to confirm the location in
Harry Potter and the Philosophers/Sorcerors Stone. As pointed out by Slytherin and Ravenclaw, other notable events of 12 September 1991 included the first Flying lesson, Harry being introduced to Oliver Wood, Neville breaking his wrist during the Flying lesson, and Draco malfoy challenging Harry to a duel. Also, Fluffy was discovered by Harry, Ron and Hermione that night, but, as Ravenclaw have pointed out, this may have been after midnight as they left the Common Room after midnight.
Slytherin, Ravenclaw and Gryffindoreach score
4 points for question 2, with no points going to Hufflepuff.
All three houses of Ravenclaw, Gryfindor and Slytherin fully answered question 3 correctly, including page references. Therefore
Ravenclaw, Gryfindor and Slytherin each score
2 points, with none going to Hufflepuff
And finally, question 4. This was quite an intriguing one. As gryffindor mentioned, all three of Harry Ron and Cedric have all had exposure to extreme cold during the second task of the Triwazard Tournament, with the lake being freezing cold, and when emerging from the lake, they were given Pepper-Up potions. However, as this included Hermione, Cho, Fleur, Krum and Gabrielle too, Gryffindor chose the correct answer, along with the other two Houses, and correctly identified that Cedrid, Harry and Ron have all been injured by dragons. All three Houses also provided the relevant page/chapter locations of the answers, so
Gryfindor, Slytherin and Ravenclaw, each score
2 points, and none for Hufflepuff in this question.
So how does this all add up?
Well, with one point from question 1, 4 points from Q2, 2 points from Q3 and 2 from Q4,
Gryffindor score a total of 9 points
Having scored all questions absolutely correctly,
Ravenclaw score a total of 10 points
Slytherin have the so-far unique situation of scoring a fractional House Point, having scored half a point in Q1, 4 points in Q2, 2 points in Q3 and 2 in Q4, giving
Slytherin a total score in this quiz of 8 and a half points
And Hufflepuff who set the quiz, score 4 points in question 1, but no points in the other questions, giving the total of
4 points fot Hufflepuff.
Which makes Ravencclaw the top scoting team in this Quiz, and therefore the team who will be setting the next quiz! Congratulations to Ravenclaw!
And well-done to all teams for some absolutely stunning knowledge of the books in this quiz, including Hufflepuff who set such fabulous questions! When seeing the questions originally I thought the scoring from the teams answering would be very low indeed, but I was proved very wrong - good going everyone!
Combining ths quiz's score with the running total, the overall house Points are as follows:
Slytherin 81 and a half
Hufflepuff 60
Gryffindor 58
Ravenclaw 40
So Slytherin have extended their lead over Hufflepuff, but the gaps between 2nd, 3rd and 4th places have all narrowed.
