I spoke to Paul the other day about this, here;s how it goes:
1. You have to have hit 125 posts and become a fully qualified wizard before getting your title, unless I presume you become a Quidditch player and/or Prefect (moderator). This is why the head girl has only 80 posts.
2. You are allowed one title in addition to your fully qualified wizard status eg
Gwared
Fully Qualified Wizard and
Maker of Wands
3. Two titles are permitted to moderators (one is their mod title), but I am unsure about this part as I'm not a mod.
4. Title applications go through Paul
Incidently, Paul is about to poll the use of teaching positions as titles, as he was going to use them as moderator titles and that should be up today.

Most of the multiple titles are breakable down into parts, for example wth Ladysnape:Fully Qualified Witch, Wife of Severus and Landlady of The Three Broomsticks
The Fully Qulified witch is what she kept after 125 post, the wife of severus bit is her custom title, and the Landlady of the Three Broomsticks is the title she has due to being Moderator.
There are a couple of folks who have a title with an "and" in it, where the bits of the title naturally go together one way or another (eg Slytherin Chaser, Lady of the Storm and Tamer of the Waves)
...No-one is X yet - If you wanted this one I'd have no problems - may be a good idea that I check if folks are OK with Teacher titles though, as at one point I think one of the Teacher titles was being considered for Moderators - we've had one non-Moderator teacher title before though, so it could be possible, but yes, I think it's going to be worthwhile doing a check in the next day or 3 to see how people feel about teacher titles first, so if you'd prefer to wait a few days there'll be a cleared answer on this one.
